Position Summary:
The Clinical Flow Coordinator is a registered nurse responsible for real-time patient flow optimization across designated service lines, including but not limited General Medicine, Hematology/Oncology, and Heart & Vascular. This position plays a critical role in coordinating bed assignments, facilitating early discharge planning, and partnering with outpatient clinics to ensure timely and appropriate inpatient admissions. The Clinical Flow Coordinator serves as a key escalation resource to frontline staff and physician teams and is instrumental in advancing UVA Health's strategic goals of patient progression, geographic co-localization, and high-quality care access. Key Responsibilities: *Proactively manages bed assignment strategies using Epic and real-time operational dashboards to ensure efficient, geographically aligned placement across service lines. *Serves as the clinical liaison between inpatient units, the Transfer Center, Emergency Department, and outpatient clinics to balance daily and elective admission volume. *Collaborates with Charge Nurses, Unit Leadership, Bed Planning, and Medical Leadership to resolve barriers to placement and discharge in real-time. *Participates in or lead patient progression huddles, including collecting and escalating information on expected discharges, discharge barriers, and placement delays. *Coordinates with assigned services to level-load direct admissions and optimize treatment timing and inpatient capacity. *Identifies patients appropriate for early-in-day discharge and support tracking of discharge readiness metrics and barriers to manage efficient flow of patients to meet service demands. *Assists in managing waitlists for OSH transfers and elective admissions, prioritizing based on acuity, service line requirements, and capacity constraints. *Escalates concerns appropriately to leadership and serve as a point of contact for after-hours or weekend admission coordination and flow support. *Documents and track root causes of patient placement or discharge delays, contributing to system improvement initiatives. *Supports consistent and equitable bed placement processes across weekdays and weekends in alignment with hospital policies and ASPIRE values. These roles are responsible for providing nursing care across various settings and specialties. Nurses and leaders in this category may rotate between departments or take on diverse duties, offering flexible and adaptive care based on the needs of patients and the institution. Individual contributors with responsibility in a clinical discipline or specialty. Typically involves diagnosing, treating, and caring for patients, and often include face-to-face interactions with patients. May also spend some time designing and implementing clinical programs, policies, and services using specialized knowledge and skills. Specialty license or certification required which is acquired through advanced education, training and/or experience. This career stream will be most commonly applied to patient care and/or scientific/laboratory technologist jobs. Requires broad knowledge of operational procedures and tools obtained through extensive work experience and require vocational or technical education. Works under limited supervision for routine situations. Provides assistance and/or may lead and training to lower-level employees. Problems typically are not routine and require analysis to understand. May lead daily operation activities. MINIMUM REQUIREMENTS Education: Bachelors degree. Advanced degree may be required. Experience: At least six years of experience. Supervisory experience required. Licensure: License to Practice as a Registered Nurse in the Commonwealth of Virginia required. PHYSICAL DEMANDS Job requires sitting/standing for prolonged periods and frequently bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to chemicals, radiation, blood/body fluids and infectious disease. Position Compensation Range: $48.38 - $64.00 HourlyBenefits
