Starting Salary Range: $77,807 - $83,365 + Office Benefit Package (On-call expenses): $50/Month subject to change

RADIAS Health is hiring a full-time TCM Manager to join our Hennepin Targeted Case Management team!

The TCM Manager will provide professional, quality program services to the clients served by Targeted Case Management services. This position is responsible for administrative and clinical supervisory work. This person will provide strong leadership with staff supervision while practicing current evidence-based strategies. This work involves the identification, development, implementation and evaluation of program policies and procedures as directed by state, federal and administrative codes, as well as professional practice standards. This work is performed with latitude for independent judgment, and action is reviewed by the supervisor through conferences, reports and observation of results obtained.

Hours: Monday - Friday 8 am - 4 pm or 9 am- 5 pm, 40 hours per week

The position is a hybrid model. Some work is done in the community with clients and other work can be done from home or in the office.

Duties & Responsibilities:

  • Clinical supervision of all staff, including discussion of daily activities with clients, coaching staff in clinical interventions, crisis management, and responding to unexpected events that occur in the office or in the community.
  • Administrative supervision of all staff, including performance evaluations, growth and development plans, and discipline.
  • Ensure compliance with all documentation requirements for the services such as timelines and quality
  • Review and approve all required documents in the client's chart in a timely manner.
  • Ensure timely filing of all court documents related to civil commitment.
  • Hiring of all positions in concert with Human Resources. Complete orientation and training with newly hired staff.
  • Coordinate staff coverage to maintain a high service standard for client care.
  • Document supervision and performance concerns for staff. Document supervision in the client chart as clinically appropriate and at least annually.
  • Develop and ensure completion and compliance of all mandated training. Identify and/or conduct any necessary individual training for direct reports.
  • Complete all paperwork, reports, and charting within expected timelines and in an organized manner.
  • Submit and approve all expenditures and mileage in the approved agency platform in a timely manner
  • Participate in required county meetings and bring appropriate information back to teams.
  • Participate in internal leadership meetings and bring appropriate information back to teams.
  • With support of the director, develop, implement, maintain and update all policies and procedures in regard to areas of accountability to ensure compliance with all regulatory agencies.
  • Provide consultation and training to all program staff regarding implementation of structures, changes in procedures, and development of interventions, pertaining to the program.
  • Evaluation and data collection regarding TCM services and client statistics for reporting purposes to the County, insurance companies, or DHS.
  • Networking with other agencies and managers regarding systemic issues
  • Advocacy with medical providers, family members, and other service providers to maintain a high quality of service for our clients.
  • Completion of program audits as required (client chart audits, staff schedules, trainings, etc.).
  • Implement corrective action from regulatory agencies as required.
  • Investigate and report suspected incidents of abuse and neglect.
  • Serve on work related community service committees, advisory boards, etc.
  • Functions and acts within the scope of established rules, regulations and legal standards, but is expected to function independent of direct supervision.
  • Maintain a pattern of regular work hours.
  • Other duties as assigned.

  • A Master's degree in the behavioral sciences or related field.
  • Minimum of 4,000 hours of work experience providing services to mentally ill persons.
  • 2,000 hours of experience or training in administration or supervision.
  • Must be Qualified Mental Health Professional: LICSW, LMFT, LPCC or LP.
