The Gerald L. Ignace Indian Health Center is seeking a qualified, caring, and trauma-informed Psychiatric Mental Health Nurse Practitioner (PMHNP). The Psychiatric Mental Health Nurse PMHNP works under the supervision of the Behavioral Health Director, in accordance with State guidelines, and Gerald L. Ignace Indian Health Center, Inc. policy and procedure. The PMHNP provides psychiatric consultation, diagnostic and treatment services, ongoing monitoring and assessment of medication effectiveness, and oversight of the long acting injection clinic.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

* Provides mental health assessment, diagnosis, treatment planning, and medication management for patients accessing psychiatric medication services;

* Actively coordinates essential patient care treatment plans with the primary care practitioner and care team, and involves the patient, family and care-giver in shared decision making, as appropriate;

* Provides direct and/or indirect patient care oriented towards maintaining health and preventing illness;

* Communicates with behavioral health and primary care clinicians regarding patient status and progress to ensure continuity of care;

* Initiates medication treatment options for patients with mental health conditions which are within the scope of practice of the practitioner;

* Prescribes medications as patient clinical condition dictates and adjusts treatment plans according to established protocol;

* Provides medication management through regular follow-up care and appointments;

* Provides basic health education, including health promotion and disease prevention (HP/DP) education;

* Works in conjunction with the Care Coordination team to establish and reinforce protocols for establishing care in the psychiatric medication management clinic;

* Serves as prescriber and oversight for the Long Acting Injection (LAI) Clinic, and works with the Care Coordinator, RN, to ensure fluid operations of the LAI Clinic;

* Establishes relationships with pharmaceutical companies;

* Participates as a member of the multi-disciplinary treatment team;

* Ensures timely clinical documentation through OCHIN EPIC medical records system, and maintains all appropriate mental health records, charts, and reports;

* Actively participates in Performance Improvement and Peer Review activities;

* Keeps updated on current clinical practices;

* Participates in quality assurance activities;

* Uses standardized tools as evidence-based guidelines and clinical decision support, as appropriate.

* Maintains any documentation required by state or facility guidelines to demonstrate compliance with supervision requirements

PLANNING ACTIVITIES:

* Maintain awareness of IHS productivity requirements and other entities.

* Participate in departmental planning efforts in order to meet performance improvement objectives

QUALIFICATIONS:

* Current psychiatric nurse practitioner/advance practice nurse prescriber licensure in the State of Wisconsin.

* Minimum three years of professional nursing experience in a clinic setting, plus two years of nurse practitioner experience.

* Certification as a Psychiatric Mental Health Nurse Practitioner or at least 2 years' experience prescribing for mental health.

* Current registered nurse licensure in the State of Wisconsin.

* Current Advance Practice Nurse Prescriber (APNP).

* Maintains active DEA.

* Current CPR certification.

EDUCATION and/or SKILLS/KNOWLEDGE:

* Minimum of a Master's of Science in Nursing (MSN);

* Annual continuing education per Wisconsin board of nursing;

* Demonstrates cultural sensitivity and clinical competence;

* Demonstrates professional manner and conduct at all times;

* Knowledge and respect of the American Indian culture and other culturally diverse populations the Gerald L. Ignace Indian Health Center serves.

OTHER REQUIREMENTS:

* Familiarity with the Indian Health Service, HRSA, AAAHC and NCQA requirements and standards of practice.

* Familiarity with Patient Centered Medical Home Transformation Manager responsibilities

* Familiarity with Quality Management and Improvement Committee initiatives including quality improvement programs, studies and reporting.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus. Noise level in the work environment is usually quiet.

Work is performed in an office environment utilizing various office equipment including, but not limited to, a computer, copy machine, fax machine, and multi-line phone. Ability to communicate effectively over the telephone, by computer and in person. Ability to perform well in a fast-paced work environment.
