Intake Case Manager CSP (Full Time)

POSITION SUMMARY:

Leads in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health coordination and prevention activities. Provide case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. Orients new clients and their families to CSP (Community Suport and Recovery Services Program) and health home services.

EDUCATION AND TRAINING REQUIRED:


  • Bachelor's degree preferred in psychology, social work, or other human services related field. AA degree or equivalent education level in the human service field required. One-year experience in community mental health preferred.
  • Ability to communicate effectively orally and written.
  • Ability to work as a member of a multi-disciplinary team

WORKING CONDITIONS:


  • Competently and consistently completes work accurately and thoroughly.
  • Ability to be a self-starter, attends to multiple tasks simultaneously, work with minimal supervision, and continually seeks new/better methods to organize/complete work.
  • Effectively accomplishes duties by working with others, including team members, other CCA staff and/or outside agencies.
  • Regularly available for work, reliable and on time for all things scheduled.
  • Ability to move within the building to meet with clients, team members or accessing supplies to attend to client needs.
  • Ability to transport clients and perform home visits.
  • Demonstrates competency in clinical/technical or administrative skills, procedures, methods, and tools required to perform the job successfully.
  • Able to perform in a fast-paced environment and meet or exceed expected service levels including health home targets.
