Case Manager - Veterans Housing Stability

As a Case Manager, you'll be on the front lines of preventing Veteran homelessness-guiding Veterans and their families from crisis to stability through compassionate support, practical resources, and meaningful advocacy. What You'll Do
Empower Veterans Through Case Management
Provide comprehensive, strengths-based case management to Veterans experiencing homelessness or housing instability Review intake assessments and identify individualized service needs Develop and monitor Housing Identification, Stability, and Single Coordinated Care Plans Partner with participants to set goals, execute participation agreements, and plan for long-term success Create thoughtful discharge plans that connect Veterans to ongoing community support
Promote Housing Stability & Self-Sufficiency
Deliver housing stability counseling and connect Veterans to rental assistance, subsidies, and housing programs Educate participants on tenant rights, fair housing laws, lease terms, budgeting, and home maintenance Support Veterans in navigating financial challenges and maintaining stable housing
Build Strong Community Connections
Coordinate referrals and service linkages, including:
VA non-compensation benefits Employment, education, and vocational services Healthcare, childcare, and behavioral health services Legal aid, protective payee services, and emergency assistance
Arrange transportation support, including bus passes and scheduled pick-ups/drop-offs
Guide Financial Growth
Ensure Excellence & Compliance
Maintain accurate, timely, and complete documentation in HMIS and client case files Track progress, outcomes, and service delivery with attention to detail and compliance standards Perform other duties as needed to support program success
What Success Looks Like
Veterans have clear, updated housing and care plans within 30 days of intake Case files are accurate, timely, and audit-ready Participants achieve housing stability and financial goals Veterans are quickly connected to essential community and VA services Services are delivered efficiently, compassionately, and on schedule
What You Bring
Bachelor's degree in social services (required) OR an Associate's degree with 4-5 years of relevant case management experience At least 2 years of experience in case management, assessments, counseling, and life skills support Experience working with Veterans strongly preferred Experience serving individuals with SUD, COD, or homelessness is a plus Valid Wisconsin Driver's License, good driving record, and current auto insurance
