Position Title: Care Navigator

Work Location: Remote - TX

Assignment Duration: 6 months

Work Arrangement: Remote


Position Summary

Over 5 years of experience working directly with the ABD (Aged, Blind, and Disabled) population in call center environments, Medicaid programs, and Managed Care Organizations (MCOs).

Skilled in supporting individuals with disabilities and vulnerable populations managing chronic or complex health conditions.


Background & Context

Role supports children, young adults, and other high-need groups requiring care coordination within Medicaid and managed care settings.


Key Responsibilities


  • Supporting individuals with disabilities and vulnerable populations managing chronic or complex health conditions.



  • Providing care coordination for children, young adults, and other high-need groups within the past three of the last five years.




Qualification & Experience


  • Required: High School Diploma



  • Preferred: BA in Social Work or Healthcare Management, LVN, CNA, CMA, RT, Pharmacy Technician



  • Over 5 years of experience working directly with the ABD population in call center environments, Medicaid programs, and MCOs



  • Demonstrated expertise in care coordination within the past three of the last five years



  • Customer service and call center experience



  • Computer knowledge



  • Insurance and MCO background




Additional Information


  • Disqualifier: No call center experience



  • Must-haves: Good attendance, call center background, worked with MCO insurance



  • Nice-to-haves: Background in Service Coordination
