Job Overview:

The Maternal Community Health Worker will primarily be maintaining a presence in the hospital and the community with patients to promote, motivate and improve healthy living. Our Maternal CHWs serve as a vital link between pregnant women, families, and healthcare providers. The program focuses on preventive healthcare, emphasizing access to essential services during pregnancy and postpartum.

Qualifications:

Required:

  • High School Diploma or equivalent

Scheduling Requirements:

  • Full-Time, Day
  • Working Location: Newark Beth Israel Medical Ctr- Newark, NJ
  • Local travel required inclusive of local churches, homeless shelters, and members residence

Essential Functions:

  • Meet patients in a hospital and conduct needs assessment using Social Determinants of Health Tool

  • Schedule weekly follow-up calls to patients

  • Assist patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members' lives

  • Assist patients with organizing their records, making follow-up appointments with Primary Care Physicians and/or Specialists

  • Help patients access resources, including quality of care, childcare services, food, housing, transportation

  • Maintain relationships with the Community Based Organizations
