Job Description

Elwyn Seeks a Community Employment Specialist I who will provide direct job development, job coaching and job support services to individuals with disabilities and overall coordination of their Elwyn (re)habilitation plan.
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Conduct client vocational profiles and other assessments based on funding source requirements
  • Counsel clients to best match their employment interests with their skills
  • Develop and provide job readiness training and job club activities to best promote client interest in employment
  • Assume a lead role in the team's development and review of client service plans
  • Systematically network to seek and develop appropriate employment opportunities that match client interests and skills
  • Conduct job analysis consistent with employer and client needs
  • Provide intensive client on-the-job training, in conjunction with a teaching plan and needs of the employer
  • Establish metrics to systematically track the progress and success of clients
  • Provide counseling and support to the client to develop interpersonal and soft skills success on the job
  • Plan for client transportation and provide travel training as needed
  • Transport clients in agency vehicles as needed
  • Identify and develop home and natural supports for individuals served
  • Develop and implement a coach fading plan which incorporates natural supports that are sensitive to the culture of the work environment
  • Anticipate and provide follow-along services for clients and their employers to maintain employment
  • Conduct client assessment, job find, placement, retention, fading levels and staff service volume according to department and funding source expectations
  • Maintain clear, active and constructive communication with the client while facilitating communication amongst the client, client home supports, client team members, funding source and monitoring entity representatives, supervisor, co-workers and potential employers
  • Provide diversity and inclusion training to community business partners as needed.
  • Serve as the point person for all communication with community employer and individual (including scheduling, notification of absences, etc.)
  • Serve as a liaison between Elwyn and community business partners, funding partners and other stake holders.
  • Complete monthly reporting based on funding source requirements
  • Complete annual training requirements as well as any additional training mandated by funding sources
  • Participate in team activities using positive approaches which will build and attain common goals
  • Perform cardiopulmonary resuscitation (CPR), safe crisis management, and medication administration using agency-trained protocols
  • Cooperate with Elwyn New Jersey and the State of New Jersey Department of Human Services, or any other authorized investigating body during an inspection, inquiry, or investigation
  • Maintain current state-mandated training and certifications
  • Follow all current policies and procedures of Elwyn New Jersey
  • Perform other duties as assigned

QUALIFICATIONS & SKILLS

  • An Associate's Degree or higher in a related field from an accredited college or university, or a high school diploma or equivalent with three (3) years of related experience.
  • One (1) year experience working with people with disabilities highly preferred
  • Non-profit human service experience highly preferred
  • Experience in workplace development, case management, and/or sales preferred
  • Flexibility in work schedule availability, as hours of work may vary, depending upon participants/job seekers, and may include work hours during evenings and weekends
  • Demonstrated ability to work effectively as part of a team
  • Demonstrated basic experience with Microsoft Office applications, including Word, and Outlook, Elwyn Referral Management System (ERMS) and report-writer experience preferred
  • Must be able to perform CPR/first aid and crisis intervention techniques using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Must successfully complete state-mandated Employment Specialist Foundations training series within 60 days of hire.
  • Must submit to a criminal background check and drug test and be determined to be not disqualified by the New Jersey Department of Human Services prior to working with individuals served
  • Must agree to have their name checked against the New Jersey Central Registry of Offenders Against Individuals with Developmental Disabilities and NJ Child Abuse Registry (CARI)
