Physical Therapist

The Physical Therapist (PT) provides skilled physical therapy services to patients in their homes on an intermittent basis. Care is delivered according to physician or authorized practitioner orders and the patient's plan of care, under the direction and supervision of the Administrator and Clinical Manager. The PT works collaboratively with the healthcare team to improve patient mobility, functional independence, and overall quality of life.

• Evaluate patient functional abilities and assist in developing and updating individualized plans of care in collaboration with physicians or authorized practitioners.
• Perform skilled physical therapy treatments and interventions as ordered.
• Complete initial evaluations, reassessments, recertifications, and discharge summaries as required.
• Monitor patient progress and communicate changes in condition or treatment needs to the physician and care team.
• Document patient evaluations, clinical notes, and progress reports in a timely and accurate manner.
• Educate patients and family members on home exercise programs, mobility techniques, and activities of daily living.
• Participate in interdisciplinary care planning, staff meetings, and patient care conferences.
• Supervise and provide guidance to Physical Therapist Assistants (PTAs) as required, including required supervisory visits.
• Complete OASIS assessments, transfers, resumptions of care, and discharge documentation in accordance with agency timelines and regulations.
• Participate in quality management, peer review, and utilization review activities as assigned.
• Maintain confidentiality of patient information and comply with HIPAA and agency policies.
• Maintain current CPR certification and adhere to agency standards of professional conduct.
• Perform additional duties as assigned
