Physical Therapy Assistant

We are seeking a dedicated Physical Therapist Assistant (PTA) to provide home health physical therapy services to patients in their place of residence. The PTA works under the direction and supervision of a licensed Physical Therapist (PT) and assists in implementing individualized treatment plans designed to improve mobility, function, and overall quality of life.

• Provide physical therapy treatments as delegated and supervised by the Physical Therapist (PT).
• Assist with patient evaluations and help prepare clinical and progress notes.
• Document therapy visits, patient progress, and updates to care plans in a timely and accurate manner.
• Educate patients and family members on home exercise programs and activities of daily living.
• Monitor patient progress and report any changes in condition to the supervising Physical Therapist.
• Collaborate with the PT and interdisciplinary healthcare team to support patient care planning.
• Participate in staff meetings, patient care conferences, and in-service training programs.
• Maintain patient confidentiality and comply with HIPAA and agency policies.
• Assist with ongoing reassessment of patient and family needs and coordinate therapy services accordingly.
• Participate in quality improvement and peer review activities as assigned.
• Maintain professional standards, time management, and accurate documentation.
• Perform additional duties as assigned.
