We are seeking a compassionate and experienced Home Health Physical Therapist to join a growing healthcare team serving patients throughout the Denver, Colorado area. In this role, you will play a critical part in helping patients restore mobility, regain strength, and improve independence while receiving care in the comfort of their own homes.

This position focuses on delivering individualized rehabilitation programs designed to improve physical function, promote recovery from injury or illness, and enhance overall quality of life. The ideal candidate will have strong clinical judgment, excellent communication skills, and a passion for helping patients overcome physical limitations and safely perform everyday activities.

As a Home Health Physical Therapist, you will evaluate patients, design personalized treatment plans, and implement evidence-based therapy interventions. You will work closely with physicians, caregivers, and interdisciplinary healthcare teams to ensure patients receive coordinated and effective care.

Your role will involve monitoring patient progress, adjusting treatment plans when necessary, and providing education and support to both patients and family members throughout the rehabilitation process.
