The Clinical Manager oversees the delivery of high-quality home health services to patients in their place of residence. This role provides clinical leadership, supervision of nursing staff, and coordination of patient care to ensure services meet regulatory standards and agency goals. The Clinical Manager works closely with physicians, nursing supervisors, and interdisciplinary teams to ensure safe, effective, and patient-centered care. This position is available during operating hours and participates in activities related to clinical operations, staff supervision, quality improvement, and compliance.

Responsibilities
  • Provide clinical supervision and leadership to nursing staff and patient care personnel.
  • Coordinate and oversee patient care services to ensure safe, effective, and high-quality care.
  • Recruit, hire, train, evaluate, and support clinical staff.
  • Assist with staff orientation, education, and ongoing professional development.
  • Collaborate with physicians, referral sources, and interdisciplinary teams to coordinate care.
  • Review patient medical records for regulatory and agency compliance.
  • Develop, implement, and maintain agency policies and procedures in accordance with federal, state, and local regulations, including Medicare and Medicaid guidelines.
  • Support and oversee the agency's Quality Management (QM) and Infection Control programs.
  • Participate in patient care conferences, staff meetings, and community education initiatives.
  • Monitor clinical documentation, patient outcomes, and service utilization.
  • Investigate and resolve patient care issues, incidents, or staff concerns.
  • Ensure availability of appropriate staffing and resources for patient care delivery.
  • Maintain confidentiality of patient information and uphold HIPAA compliance.
  • Assist with clinical coverage during staff shortages and participate in on-call rotation when necessary.
  • Perform additional duties as assigned by leadership.
