Care Management Support Coordinator II Position Purpose Supports administrative care management activities including outreach, inbound calls, and scheduling services. Acts as a point of contact for members, providers, and staff to resolve issues and documents member records in compliance with state and regulatory guidelines. Education & Experience * High School diploma or GED required * 2-3 years of related experience in managed care, medical office/facility, or call center Additional Skills & Qualifications * Required: Spanish speaking skills * Experience: Call center and medical administration background * Knowledge: Medical terminology in Medicare/Medicaid field * Preferred: Referral knowledge with local organizations that support social service needs * Disqualifiers: Lack of call center experience or medical knowledge Day-to-Day Responsibilities * Perform outreach calls to members to support care plan next steps. * Answer inbound calls and assist with scheduling services * Provide referrals to health plan and community resources for social service needs (clothing, food, financial, family, medical) * Support member onboarding with welcome letters, correspondence, and educational materials * Document and maintain non-clinical member records per regulatory requirements Job Type & LocationThis is a Contract position based out of Tempe, AZ. Pay and BenefitsThe pay range for this position is $23.00 - $23.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
