Job Summary
The Community Health Project Supervisor supports community resource engagement across the health system, the successful expansion of the Findhelp referral network assists in the coordination, implementation and performance of community health related activities, events and workshops. The position serves as a link between Montefiore and the community and acts as a community source of information for MMC community services. Performs related duties as required. Schedule: 9am-5pm with some weekend and evening coverage
Essential Functions
- Assist in developing and implementing of Community Health Education programming and other program initiatives.
- Develop and maintain relationships with key community leaders, providers, community members, and other relevant groups within the service area.
- Attend health fairs, community events and seminars; solicit involvement of other MMC departments in community events; provide logistical support and departmental event planning;
- Attend departmental and community meetings as assigned.
- Support management of vendor relationship with Findhelp as it relates to community engagement and provide operational and technical support to MHS community engagement teams to recruit local CBOs to list and regularly update programs in Findhelp, as well as adopt workflows to accept and respond to patient referrals and document outcomes.
- Support the development of a library of streamlined community education content that can be used across MHS programs.
Qualifications
- High School Diploma or GED Required
- Bachelors in Health Education, Community Health or Public Health Preferred
- 1-3 years Health promotion, education training; Community Engagement experience Required
- Bilingual: English/Spanish, a plus.
- Basic knowledge of computer programs; Microsoft Word, PowerPoint, and Excel.
- Excellent presentation and interpersonal skills.
