JOB DESCRIPTION:

SUMMARY OF POSITION

Provides non-clinical support to individuals with substance use and co-occurring disorders needs by facilitating access to treatment, recovery support, and community resources. This role focuses on engagement, care coordination, and continuity of care while upholding ethical standards, client choice, and regulatory requirements.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Serve as a point of contact for individuals seeking recovery-oriented services and assist with navigation of appropriate levels of care.
  • Support timely linkage to treatment, recovery housing, peer support and community-based resources in accordance with individual needs and eligibility.
  • Coordinate with internal clinical teams, case managers, and external partners to support continuity of care and successful transitions.
  • Provide accurate, unbiased information about available services while honoring client autonomy and informed choice.
  • Assist individuals in understanding program expectations, intake processes, and referral pathways.
  • Maintain appropriate documentation in accordance with organizational policies, accreditation standards, and confidentiality regulations.
  • Participate in multi-disciplinary team meetings to support client engagement and retention in services.
  • Identify barriers to access (transportation insurance, housing).
  • Support quality improvement efforts by tracking engagement activities and outcomes assigned.

Education Level

High School diploma or equivalent with experience in health care marketing/ community engagement. Bachelor's in social service or related field preferred.

Years of Experience

Minimum of two years' experience providing outreach and community engagement for a healthcare organization.

Pay Range:

$19.10--$26.80
