Title: Antimicrobial Stwd Pharmacist Location: Fountain Valley Department: Pharmacy Administration Status: Full Time Shift: Days Pay Range*: $81.34/hr - $117.94/hr Under the general supervision of the Clinical Pharmacy Program Manager, the Antimicrobial Stewardship (AMS) Pharmacist serves as a clinical leader responsible for optimizing antimicrobial use across MemorialCare. The AMS Pharmacist provides comprehensive pharmaceutical care with a focus on evidence based management of infectious diseases and systemwide stewardship initiatives. The AMS Pharmacist will develop, implement, and evaluate antimicrobial utilization management programs; lead efforts to ensure appropriate antimicrobial selection, dosing, monitoring, de escalation, and duration of therapy; and actively participate in patient specific infectious diseases pharmacotherapy decisions. The pharmacist will contribute to the facility's teaching and research mission by providing education to physicians, nurses, pharmacists, and trainees on principles of optimal antimicrobial use and resistance prevention. This role will also participate in departmental and interdisciplinary committees, collaborate closely with microbiology and infection prevention teams, and support surveillance, guideline development, and policy enforcement related to antimicrobial use. Acting as a liaison across departments and campuses, the AMS Pharmacist ensures consistent, safe, and effective antimicrobial practices that reduce resistance, prevent medication related harm, and advance MemorialCare's systemwide stewardship goals. The incumbent will also serve as a liaison between the pharmacy department and Best Practice Teams (BPTs), ensuring that pharmacy provides measurable and clear value to the BPT process. Additional responsibilities include identification, development and management of clinical cost savings initiatives, collaborate with other clinicians, stakeholders, and committees to successfully develop and implement policies, procedures, practice standards, and therapeutic guidelines. Essential Functions and Responsibilities of the Job List of broad job specific functions that employees must be able to do: 1. Develop, implement, and evaluate antimicrobial stewardship initiatives to optimize antimicrobial selection, dosing, monitoring, deescalation, and duration of therapy across the health system, with an emphasis on positive patient outcomes and cost effective therapy. 2. Conduct antimicrobial utilization review and reporting, monitoring prescribing trends, resistance patterns, and medication related safety issues. 3. Support and participate in system and local AMS committee meetings, including Best Practice Team (BPT) meetings; ensuring alignment, communication, and consistent stewardship practices across campuses. 4. Provide infectious diseases pharmacotherapy support, including expert consultation on antimicrobial selection, dosing, and monitoring. 5. Develop and maintain systemwide antimicrobial guidelines, policies, and order sets in collaboration with interdisciplinary teams. 6. Deliver education to clinicians and staff on antimicrobial stewardship principles, resistance prevention, and optimal antimicrobial use. 7. Be at work and be on time 8. *Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
