Description
What You Will Do to Change Lives At Telecare's AgeWise program, we provide community-based, strengths-based case management and institutional care coordination for older adults (aged 59+). As a Case Manager III, you will serve as a primary support advocate, helping individuals with mental illness and co-occurring disorders navigate complex systems, build comprehensive care plans, and live as independently as possible in their community.
Shifts Available: Full-Time | 8:00 AM - 5:00 PM | Monday - Friday Compensation: Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. AgeWise is a community-based program serving older adults. AgeWise serves a maximum of 600 individuals, aged 59+. Agewise's Strengths-Based Case Management is a consumer-driven and highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care and natural supports. Agewise's Institutional Case Management (ICM) serves members who are conserved and placed in a long-term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure that they are placed in the least restrictive environment possible.
