We arelooking fora licensed social worker witheffective communicationskills,the ability to effectively engage patients in a therapeutic relationship,working knowledge of differential diagnosis of common mental health and/or substance use disorders. andknowledge of psychopharmacology for common mental health disorders.

TheCare Managerfunctions as a core member of a collaborative care team that involves the patient's primary care provider, psychiatric consultant, and other mental health providers. The Care Manageris responsible forcoordinating and supporting mental health care within the care setting and for coordinating referrals to clinicallyindicatedservices outside the care site.

Primary Duties:

  • Completesrelapseprevention plan with patients who are in remission.Documents patient progress and treatment recommendations in the registry so they can be easily shared withPCPs (Primary Care Physicians), psychiatricconsultants,and othertreatmentproviders.Monitors patients in person or by phone for changes in clinical symptoms, treatment side effects, or complications.

  • Facilitates patient engagement and follow-up care. Tracks patient follow-up and clinical outcomes. Documentsencounterin a registry and use the system toidentifyand re-engage patients.Facilitates referrals for clinicallyindicatedservices outside the primary care clinic such as social services, housingassistance, vocational rehabilitation, mental health specialty care, or substance abuse treatment.Facilitates treatment plan changes for patients who are not improving as expected in consultation withPCPand psychiatric consultant. This may include changes in medications or psychosocial treatments withappropriate referralsforadditionalservices.

  • Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient's primary care provider.Consultations will focus on patients new to treatment or those who are not improving as expected.Supports and closely coordinates mental health care with the patient's primary care provider and, whenappropriate, other mental health providers.

  • Providesbrief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving treatment,motivationalinterviewing,or other treatmentsappropriate forprimary care settings.Screens and assesses patients for common mental health and substance abuse disorders.Providesbasic education and available treatment options.Providesreferrals to evidence-based psychosocial treatments as clinicallyindicated.

Qualifications:

  • Master's degree in psychology, SocialWork,or related field.

  • BSN orLPN (Licensed Practical Nurse)with mental health training/certification may be substituted.

  • Experienceworking with patients who have co-occurring mental health, substanceabuseand physical health problems.

  • Experience screening for common mental health and/or substance use disorders patient assessments and treatment planning Required and

  • LCSW-C (Licensed Certified Social Worker-Clinical), LMSW (Licensedmaster'sSocial Work) or LCPC (Licensed Clinical Professional Counselor).

  • PhD in Psychology may be substituted

  • 1-2 years Related experience in a similar care modelpreferred.
