Come join our dynamic, highly skilled UCSF Geriatric Inpatient Services APP team! The UCSF Geriatric Inpatient Services include the Acute Care for Elders (ACE) Program and Inpatient Geriatrics Consultation, provided at UCSF Medical Center at UCSF Stanyan hospital. This position will be primarily based at the Stanyan Hospital, with cross-training to cover UCSF Parnassus. We are seeking a highly motivated Gerontology Nurse Practitioner (NP) to play a pivotal role in delivering patient-centered care for older adults and to serve as a Nursing leader in UCSF Health's innovative new ACE Program at Stanyan Hospital. The role includes direct patient care, inspiring and engaging nursing staff, and leading quality improvement initiatives to create and improve programs that enhance patient experiences and outcomes. Responsibilities also include developing an educational program to increase nursing and other disciplines geriatrics competencies across ACE units and the broader health system, participating in quality improvement initiatives, and expanding geriatrics programs into key areas such as the Age-Friendly Health System and other hospital units.

Under the supervision of the Medical Director and guided by UCSF Health policies and procedures, the APP position offers a broad scope of practice, including management of chronic stable medical conditions, specialty protocols, routine care, acute or critical care, and medical and surgical procedures, per approved standardized procedures. The APP will provide high-quality care to adult and geriatric patients across UCSF Health licensed settings. 4. Ability to cope with personal stress experienced by health care providers and team members
5. Ability to meet minimum standards for hospital credentialing and billing standards as a billable provider

6. Submit health plan (Medicare, Medicaid, and commercial for all APPs; also, CCS for pediatric providers) enrollment by first clinical day

7. At all times, must adhere to the UCSF Medical Staff Bylaws, Rules and Regulations, Code of Conduct, PRIDE values, and applicable medical center/campus/departmental policies and procedures

8. At all times, must adhere to all UCSF policies regarding HIPPA, privacy, communication, documentation, encounter closure, and billing

9. Mandatory attendance at the Office of Advanced Practice Providers Orientation Sessions

10. Mandatory attendance at the APP Onboarding Committee APP Onboarding Series

11. This position requires flexibility for the APP to work at all UCSF Health facilities/locations

12. Able to perform and interpret cognitive assessments

13. Able to perform and interpret functional assessments

14. Able to recognize and de-prescribe high-risk medications

15. Performs other duties as assigned

Requirements:

Graduation from a California Board of Registered Nursing recognized Nurse Practitioner Program with a master's or doctoral degree aligned with the patient population and clinical role.

  • A minimum of six (6) months' experience as a Gerontology Nurse Practitioner

Preferred Qualifications:

  • Experience in nursing leadership and program management
  • Experience developing and implementing strategies to prevent cognitive and functional decline in acutely hospitalized older adults
  • Minimum of 3 years of acute inpatient geriatric experience.
  • Minimum of 1 year working as a Gerontology NP

License/Certifications:

1. California Registered Nurse (RN) License without restriction
2. California Nurse Practitioner (NP) License without restriction
3. California Nurse Practitioner Furnishing Number
4. NP national board certification from an accepted certifying agency aligned with the patient population and clinical role.
Adult Inpatient: (ACNP-BC, AGACNP-BC, ACNPC-AG, ACNPC).
5. California Driver License (if required for position)
6. Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address
7. American Heart Association (AHA) Basic Life Support (BLS) certification
8. Additional required AHA certifications: ACLS
9. Additional required certifications (such as ENLS): None

Requirements:
