We are seeking a Paramedic to join Tufts Medicine growing Mobile Integrated Health team. In this role you will provide primary care and advanced life support, medical evaluations, treatment and stabilization of Lowell General Hospital patients with chronic and acute health issues in their homes in the Lowell area. Position Overview: This position is an allied health professional who possesses the complex knowledge and skills necessary to provide patient care in the home and institutional setting in an effort to decrease emergency department and hospital utilization. Functions as part of a comprehensive mobile integrated response, under medical oversight and conduct detailed assessments and available diagnostics to collaborate with the medical control physician to determine appropriate treatments and intermediate care management strategy for patients with acute and chronic conditions. Responds to patient care needs, requests and places appropriate referrals, and provides education to patients and their families regarding their medical conditions. Conducts patient care visits with real-time communication with one of our board-certified emergency physicians or the patient's attending physician and works cooperatively within the healthcare system and other services to create a system of quality health care. Location: Lowell General Hospital - 295 Varnum Avenue - Lowell, MA Hours: 36 hours, Variable Shifts (Days/Evenings/Weekends/Holidays) Minimum Qualifications 1. Completion of accredited Paramedic program. 2. National Registered Paramedic. 3. Massachusetts Certified Paramedic. 4. Basic Life Support (BLS) certification. 5. Advanced Cardiovascular Life Support (ACLS) certification. 6. Valid State issued driver's license. 7. Two (2) years of relevant experience. 8. Must obtain Community Paramedic certification by the International Board of Specialty Certifications (IBSC) within one (1) year of start of employment. Preferred Qualifications: 1. Bachelor's degree. 2. BCLS, ACLS, PALS Instructor. 3. Incident Command System (ICS) course up to 200 level. 4. Pediatric Advanced Life Support (PALS) certification. 5. Three (3) years of experience in community paramedicine or mobile integrated health. Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned. 1. Complies with performance improvement clinical benchmarks. 2. Answers phone and places calls as needed using proper professional etiquette. 3. Completes patient care documentation required in the various Tufts Medicine electronic medical record systems. 4. Avoids hazards, uses safety equipment. 5. Follows clinical protocols, plans, policies and physician orders correctly. 6. Performs peer review QA when assigned and participates in department M&M rounds by reviewing cases when requested. 7. Attends a minimum of six (6) department M&M rounds per year unless granted an exemption. 8. Follows operational flow and procedures efficiently and effectively to maximize utilization of the Mobile Integrated Health Program. 9. Works collaboratively and in a professional manner with all allied health and public safety personnel. 10. Draws labs on patients as ordered and documents in hospital EMR (Epic). 11. Maintains supplies and clean and orderly equipment on mobile units. 12. Performs 12 Lead EKGs, ultrasound imaging, simple dressings, wound care, ace wraps, ADL in the home and institutional setting as needed, facilitates telehealth visits with providers. 13. Works collaboratively and in a professional manner with all allied health, case managers, physicians to coordinate appropriate resources that are part of the Tufts Medicine continuum of care. 14. Participates in interdisciplinary rounds and patient care huddles when necessary. 15. Performs home visits (scheduled, unscheduled, urgent), assessments and treatments according to Mobile Integrated Health protocols and procedures. 16. Understands and participates in care planning with the patient's physicians, including referrals to other care providers such as VNA, PT and OT. Lifestyle, dietary and medication coaching. 17. Completes within one year of employment all necessary training required to operate in an expanded scope of practice which includes: advanced wound assessment, initiation and management of urinary catheters, central line management, chronic disease management with early goal directed therapy and other assigned competencies. Physical Requirements: 1. Prolonged, extensive, or considerable standing/walking, and climbing stairs. 2. Lifts, positions, pushes and/or transfer patients and equipment up to 125 pounds. 3. Considerable reaching, stooping, bending, kneeling, crouching, or crawling. 4. Ability to use hands to finger, handle, or feel objects, tools or controls 5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections. 6. May have contact with hazardous materials. 7. Specific vision abilities apply including close vision, distance vision and ability to adjust focus. 8. Subject to varying and unpredictable situations, including weather conditions. 9. Ability to handle emergency and crisis situations. Skills & Abilities: 1. Demonstrated organizational skills and attention to detail. 2. Ability to prioritize work and complete tasks in a timely manner. 3. Ability to work independently and accurately with excellent follow-through. 4. Basic to moderate computer skills including the ability to navigate through hospital-based computer systems. 5. Knowledge of medical terminology. 6. Knowledge of math, ability to make calculations and analytical skills. 7. Communicates with others (including patients) and must effectively be able to exchange information accurately via phone, email, and or in person. 8. Ability to maintain sensitive and confidential information. 9. Skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. This includes: Knowledge of growth and development; Ability to interpret age specific data and response to care; and Provide age specific communication. 10. Ability to navigate with the assistance of maps and GPS. 11.Demonstrates competence in use of assigned equipment and has established competence in safe ambulance operations. Why Join Us?
