$3,000 for LCSW and $10,000 for LICSW Starting rates: LCSW: $68,224 LICSW: $92,227 We are seeking a dynamic individual with strong clinical, collaborative, and consultation skills to join our Transplant Social Work team. In this role, you will support the Kidney/Pancreas Transplant Program by conducting comprehensive psychosocial assessments to determine candidacy for kidney and/or pancreas transplantation and presenting your recommendations to the multidisciplinary Selection Committee. You will provide ongoing support and care coordination to patients on the transplant wait list as they prepare themselves and their support systems for transplantation. The ideal candidate will have experience in conducting psychosocial assessments, group facilitation, crisis intervention, and interprofessional collaboration. They will also have experience working with patients and families presenting with a wide range of mental health and psychosocial concerns including anxiety and depression, adjustment and coping, loss and bereavement, complex family dynamics, substance use disorder and trauma. Excellent clinical, communication, and documentation skills are a necessity. Summary This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community. Does this position require Patient Care? Yes Essential Functions -Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
