Department/Position Summary:
CARES for Kids is a MassHealth program to support clinics that care for children with medical complexity. This program will be available for solid organ transplant patients in the Pediatric Transplant Center (PTC) who are MassHealth members under 21 years old who meet the medical necessity criteria. This RN will be responsible for leading the development, implementation and oversight of the coordination for Boston Children's Hospital CARES for Kids Program within the PTC. The RN will serve as the clinical expert and strategic resource to get eligible transplant patients enrolled in the program, work with the centralized CARES program at BCH and coordinate with patients and families. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
