Description IRIS Consultant Case Manager \| Long-Term Care Consultant \| Disability Services Coordinator Milwaukee, WI (Hybrid) Full-Time \| 40 Hours/Week \| Monday-Friday \| 8:00 AM-5:00 PM Travel Required (30-50%) Benefits Eligible Empower Individuals to Live Independently Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services. As an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose. What You'll Do As an IRIS Consultant, you will: Develop and maintain trusted relationships with IRIS participants Assist participants in creating and updating individualized IRIS Support and Services Plans Conduct monthly participant contacts and required quarterly in-person visits Promote participant choice, self-direction, independence, and community inclusion Monitor participant health, safety, and program outcomes Educate participants about IRIS enrollment requirements, eligibility, and available resources Collaborate with Aging and Disability Resource Centers (ADRCs), healthcare providers, community organizations, and service providers Maintain accurate and timely case documentation within required systems Complete documentation within established compliance standards and timelines Assist with annual Long-Term Care Functional Screens when applicable Work independently while collaborating with an interdisciplinary support team Qualifications Required One of the following: Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Special Education, Nursing, Rehabilitation Services, Healthcare Administration, or a related field and at least one year of supervised experience working with older adults or individuals with disabilities OR Four (4) or more years of direct experience providing services to older adults or individuals with disabilities OR Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption Preferred Skills & Experience Experience in any of the following is highly desirable: Case management Care coordination Disability services Long-term care Social work Human services Aging services Community-based services Medicaid waiver programs Behavioral health Independent living services Person-centered planning Healthcare navigation Family support services Additional Qualifications Strong communication and relationship-building skills Ability to work independently and manage a flexible schedule Proficiency with smartphones, tablets, and computer systems Commitment to participant rights, dignity, and self-determination Ability to serve diverse populations with cultural awareness and respect Bilingual Candidates Encouraged to Apply Fluency in any of the following languages is highly valued: Spanish Hmong Arabic Russian American Sign Language (ASL) Other languages commonly spoken in Wisconsin communities Benefits LSS offers a comprehensive benefits package including: Medical, Dental, and Vision Insurance Flexible Spending Accounts (Health & Dependent Care) Paid Time Off (PTO) 10 Paid Holidays Mileage Reimbursement 403(b) Retirement Plan Employee Assistance Program (EAP) Annual Wage Increases Calm Premium Wellness App UKG Wallet Early Wage Access Service Awards and Employee Recognition Public Service Loan Forgiveness (PSLF) Eligible Employer PSLF Education and Support Resources Requirements Valid driver's license Reliable transportation Ability to travel throughout Milwaukee and surrounding counties Satisfactory driving record consistent with LSS Driver Safety requirements Ability to meet organizational automobile insurance standards Join Our Growing IRIS Team If you're passionate about social services, case management, disability advocacy, long-term care, independent living, healthcare coordination, and helping people achieve their goals, we'd love to hear from you.
