Location: Calmont Operations BuildingDepartment: Healthy HomesShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: Functions in collaborative practice with families, school personnel, dental/medical professionals and community partners involved with The Center for Children's Health, community agencies, volunteers and co-workers in Cook Children's Health Care System. Primarily responsible for providing preventative dental/health/safety education, family resources and support services to targeted children and families. Facilitates and organizes access to oral/medical health care and/or social services for an identified group of children/families. The Community Health Worker / Promotor(a)s focus will be to assist children and their families in making healthy lifestyle choices that will lead to an improved quality of life and better overall health. Skillfully and consistently performs duties per established policies and procedures. Requires routine travel in Tarrant County with possible travel to neighboring counties as needed. Qualifications:
