Summary:

Functions in collaborative practice with families, school personnel, dental/medical professionals and community partners involved with The Center for Children's Health, community agencies, volunteers and co-workers in Cook Children's Health Care System. Primarily responsible for providing preventative dental/health/safety education, family resources and support services to targeted children and families. Facilitates and organizes access to oral/medical health care and/or social services for an identified group of children/families. The Community Health Worker / Promotor(a)s focus will be to assist children and their families in making healthy lifestyle choices that will lead to an improved quality of life and better overall health. Skillfully and consistently performs duties per established policies and procedures. Requires routine travel in Tarrant County with possible travel to neighboring counties as needed.

Qualifications:

  • High School diploma or GED required; Associates preferred

  • TX DSHS Community Health Worker (CHW)/Promotora(es) certificate required

  • Required CE hours to maintain current CHW certification

  • Ability to work autonomously with little supervision

  • Familiarity with basic dental/medical/safety terminology

  • Bilingual in English and another language preferred

  • Minimum 1-year experience in coordinated care or outreach activities required

  • Current CPR certification; or at least required within 90 days of employment

  • Current, valid Texas driver license with clean driving record

  • Take and pass Cook Children's Defensive Driving course, once employed and annually thereafter

  • Proof of automobile liability insurance (at level required by Cook Children's)

  • Able to drive assigned CCHCS vehicle as an approved driver

  • Must be at least 21 years old

  • Healthy Homes: Must have private, reliable transportation and be willing to travel within Cook Children's six-county area.

  • Certification as technician in child passenger safety by Safe Kids/NHTSA required or must be obtained within 12 months of hire.
