POSITION SUMMARY

Seeking a skilled group facilitator to work in a psychiatric rehab setting. Excellent opportunity for an individual looking for dynamic position outside of clinic/hospital setting with daytime hours and no weekends. The program provides individuals with severe and persistent mental health conditions a full range of comprehensive, recovery-oriented services, with the purpose of assisting them in overcoming barriers caused by their mental health condition, developmental disability or substance use disorder.

This role serves as part of a multi-disciplinary team providing group facilitation, individual services, relapse prevention and crisis intervention.

Essential Responsibilities

Program Operations



  • Assist in management/program operations in collaboration with PROS Director.
  • Assist in providing complete oversight of the PROS program including but not limited to class and staffing coverage, coordination of services, general daily operations, and managing routines and events.
  • Assist Program Director in the creation, maintenance, and ongoing development of PROS curriculum.
  • Collaborate with Program Director and PROS Team to assist in Utilization Review, Quality Assurance, and Compliance activities.
  • Function as a leader and provide expertise and innovation for areas of need for enhanced service provision in PROS program and throughout the agency.


Staff Oversight



  • Oversee PROS Facilitators for the purpose of coordinating program operations in conjunction with and in the absence of Program Director.
  • Provide guidance and support to members of the PROS Facilitators team. Assist team in managing day-to-day operations and program needs.


PROS Group Facilitation and Direct Care



  • Facilitate psychiatric rehabilitation groups including wellness self-management services designed to develop or improve individual's personal coping skills and basic living skills training designed to improve individual's ability to perform the basic skills critical to his/her recovery.
  • Collaborate with individual and collaterals in developing, reviewing and modifying an Individualized Recovery Plan that identifies the medically necessary services that will support the individual's goals and identified path to recovery.
  • Maintain electronic case records in a timely manner and complying with OMH and Medicaid regulations and agency policies and procedures.
  • Coordinate the discharge planning process. Identify the resources and supports needed for transition to another program and make necessary referrals, linkages for treatment, rehabilitation and support services.


Health, Safety, & Compliance



  • Maintain a work environment in a clean, organized, and safe manner. Conduct daily inspections of work areas to look for unsafe conditions
  • Use all required safety devices and personal protective equipment.
  • Ensure all accidents are reported to supervisor and Human Resources.
  • Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.

  • Maintain a clean and neat appearance consistent with agency standards and act as a role model
  • Attend all required meetings and trainings
  • Participate in training programs for professional growth as scheduled and approved
  • Responsible for understanding and adhering to Gateway Community Industries' Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies. Keep abreast of any changes or modifications to policies or regulations. Ensure that all staff are knowledgeable of applicable regulations, laws, and policies
  • Perform other duties as assigned

Qualifications

REQUIRED QUALIFICATIONS

Education:



  • Bachelor's Degree plus 1 year experience in related field OR
  • Associate's degree plus 2 years' experience in related field


Experience:



  • 1-2 years' experience in providing direct service related to the care or treatment of persons with a designated mental illness diagnosis
