What you will do:

The Clinical Case Manager II functions as an independent practitioner providing comprehensive psychosocial assessment, treatment planning, and discharge coordination for patients in the inpatient psychiatric setting. This role manages a full caseload of moderate to high complexity patients, including those with co-occurring disorders, psychosocial barriers, and complex discharge needs. Working collaboratively with the interdisciplinary team, the Clinical Case Manager II is responsible for driving treatment planning, resolving barriers to discharge, and ensuring efficient transitions of care. This role demonstrates advanced clinical judgment in crisis intervention, therapeutic modalities, and systems navigation.

In addition to direct patient care, the Clinical Case Manager II contributes to the development of others by mentoring staff and supporting team-based problem solving. The role also participates in quality improvement initiatives and strengthens partnerships with community providers to enhance continuity of care.

Clinical Services

• Conduct comprehensive psychosocial assessments and independently develop, implement, and adjust individualized treatment and discharge plans.

• Manage a full caseload of moderate to high complexity patients, including those with co-occurring disorders, psychosocial instability, and discharge barriers.

• Provide crisis intervention and evidence-based therapeutic services across individual, group, and family settings.

• Lead discharge planning efforts, proactively identifying and resolving barriers related to housing, insurance, legal status, and access to care.

• Coordinate complex transitions of care, ensuring continuity across inpatient, outpatient, and community-based services.

• Ensure full compliance with regulatory, legal, and accreditation standards, including patient rights, mandated reporting, and documentation requirements.

• Independently manage court coordination and legal processes, ensuring accuracy and timeliness.

• Maintain high-quality documentation that reflects clinical decision-making and patient progress.

Interdisciplinary Collaboration

• Actively contribute to interdisciplinary team meetings, providing clinical insight and recommendations that influence treatment direction.

• Collaborate with internal and external providers to resolve care coordination challenges and improve discharge outcomes.

• Maintain and leverage strong community partnerships to support complex discharge needs.

Supervision

• Mentor and support onboarding of new staff, promoting competence in clinical care, documentation, and workflows.

• Provide clinical supervision to lower-level staff, including oversight of documentation and evaluation of performance.

• Serve as a resource to peers for clinical and process-related questions.

Professional Development/Community Education

• Participate in quality improvement, safety, and performance initiatives.

• Contribute to committees, task forces, or projects that support departmental goals.

• Maintain current knowledge of best practices and apply them to clinical work.

• Develops and delivers community education programs and workshops that increase awareness of behavioral health issues and available resources.

What you will need:

• Education: Masters Degree in Social Work or Master's Degree in a recognized mental health field

• License : Current State of Illinois licensure with a minimum of four letters e.g. LCSW (Licensed Clinical Social Worker), LCPC (Licensed Clinical Professional Counselor), LMFT (Licensed Marriage and Family Therapist) or Clinical Psychologist (Psy D/Ph D).

• Certification: Current CPR Certification issued by either American Heart Association or Red Cross within 90 days of hire. CPI (Crisis Prevention Intervention) certification within 90 days of hire.

• Experience: Minimum of 2 years of experience in a behavioral health environment.
