Hourly Pay Range: $32.60 - $48.90 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:
What you will do: The Clinical Case Manager II functions as an independent practitioner providing comprehensive psychosocial assessment, treatment planning, and discharge coordination for patients in the inpatient psychiatric setting. This role manages a full caseload of moderate to high complexity patients, including those with co-occurring disorders, psychosocial barriers, and complex discharge needs. Working collaboratively with the interdisciplinary team, the Clinical Case Manager II is responsible for driving treatment planning, resolving barriers to discharge, and ensuring efficient transitions of care. This role demonstrates advanced clinical judgment in crisis intervention, therapeutic modalities, and systems navigation. In addition to direct patient care, the Clinical Case Manager II contributes to the development of others by mentoring staff and supporting team-based problem solving. The role also participates in quality improvement initiatives and strengthens partnerships with community providers to enhance continuity of care. Clinical Services
