MaineHealth is seeking a compassionate and highly skilled Post Transplant Coordinator (PoTC) to join our multidisciplinary transplant team. In this role, you will coordinate and manage patient care following solid organ transplantation, serving as a vital link between transplant recipients, transplant nephrologists, surgeons, and other members of the care team.

The Post Transplant Coordinator is responsible for ensuring continuity of care across the transplant journey, providing patient education, monitoring clinical progress, coordinating follow-up services, and promoting exceptional outcomes for transplant recipients. You will play a critical role in supporting patients as they navigate life after transplantation while helping deliver high-quality, patient-centered care.

The position works collaboratively with the transplant team within the MaineHealth Transplant Program Portland.

Summary:

The Post Transplant Coordinator (PoTC) role is responsible for coordinating patient care after transplants, working in collaboration with the transplant nephrologists and transplant surgeon, and participating as a member of the multi-disciplinary solid organ transplant team.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: Bachelor's Degree preferred.
  • License/Certifications: Maintains licensure and certification respective of profession.
  • Experience: RN, PA, NP, recent critical care, or other related experience required.

The pay range for this position is $89,980.80to $121,721.60per year.

Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
