Registered Nurse - Wound Care - Woodridge Rehabilitation and Nursing

The Wound Care Nurse serves as the clinical leader for the facility's wound care and skin integrity program, supporting both short-term rehabilitation and long-term care residents. In this role, you will oversee the prevention, assessment, treatment, and healing of wounds while promoting resident-centered care and ensuring compliance with regulatory and quality standards. As the wound care expert, you will collaborate with interdisciplinary teams to develop and implement individualized treatment plans that optimize healing outcomes and enhance residents' quality of life.

Key responsibilities include conducting comprehensive skin and wound assessments, performing wound measurements, documenting and monitoring healing progress, and entering and managing wound care orders within the electronic medical record. The Wound Care Nurse works closely with physicians, nurse practitioners, rehabilitation staff, dietitians, and nursing teams to coordinate care and drive positive clinical outcomes. This position also plays a critical role in pressure injury prevention, quality improvement initiatives, staff education, and competency development. The ideal candidate is a skilled clinician with strong assessment and communication abilities, sound clinical judgment, and a passion for evidence-based practice, continuous improvement, and exceptional resident care.

DAY SHIFT: 0900 - 1430. Scheduled days of the week negotiable.

Position Specific Requirements

• Minimum of two to three years of nursing experience in long-term care, skilled nursing, rehabilitation, medical-surgical, wound care, or a related clinical setting.
• Demonstrated experience in wound assessment, pressure injury prevention, treatment planning, and interdisciplinary care coordination.

• Wound care certification (WCC, WOCN, CWCN, or equivalent) preferred, or willingness to obtain certification within one year of hire.
• Strong knowledge of wound assessment and treatment, pressure injury staging, skin integrity management, and evidence-based wound care practices.
• Proficiency with electronic medical records (EMR) and accurate clinical documentation.
• Understanding of regulatory requirements, quality improvement processes, and infection prevention standards within a long-term care or rehabilitation setting.
• Ability to work independently, prioritize multiple responsibilities, and exercise sound clinical judgment.
• Excellent communication and interpersonal skills, with the ability to effectively collaborate with residents, families, providers, and interdisciplinary team members.
• Ability to educate, mentor, and support nursing staff while serving as a clinical resource and wound care expert for the organization.
