RN Case Manager

The Department of Pediatrics at the University of Virginia's School of Medicine seeks applicants for an RN Case Manager to support Blue Ridge Care Connection for Children. This position is ideal for a dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team. This registered nurse utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care.

The successful candidate will perform the following duties:

• In addition to the above job responsibilities, other duties may be assigned.
• Asses the physical, functional, social psychological, environmental, learning and financial needs of patients.
• Identify problems, goals, and interventions designed to meet outpatient needs, including prioritized goals that consider the patient/caregivers goals, preferences and desired level of involvement in the case management.
• Provide appropriate interventions which demonstrates knowledge of and sensitivity towards cultural diversity and religious beliefs, developmental, health literacy, and educational background of the population served by Blue Ridge Care Connection for Children.
• Utilize interpreter services per policy.
• Provide referrals to appropriate community resources: facilitate access and communication when multiple services are involved.
• Monitor activities to ensure that services are being delivered and meet the need of the patient, coordinate services to avoid duplication.
• Communicate with primary care physicians and members of comprehensive care team regarding status of patient.
• Maintain accurate patient records and patient confidentiality.
• Case Management Certification must be obtained within five years of hire and maintained during employment.
• In addition to the above job responsibilities, other duties may be assigned.

MINIMUM REQUIREMENTS

Education: Graduate of an accredited nursing program required.

Experience: 1 year of relevant experience required.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.

Preferred Qualifications:

• BSN
• National certification in case management
• Fluent in Spanish

Knowledge, skills and abilities:

• Knowledge and skill chronic disease management of children with special needs
• Excellent verbal and written communication including documentation skills.
• Excellent relationships management with patients, families and care providers.
• Ability to work as a member of a team.
• Exceptional customer services skills and understanding of patient and family-centered care concept.
• Proficient in Microsoft office.
• Experience using Electronic Medical Record preferred.
• Self-directed
• Ability to promote/participate inpatient center medical home mode of care.
• Strong organizational skills

PHYSICAL DEMANDS

Job requires sitting/standing for prolonged periods and frequently bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to chemicals, radiation, blood/body fluids and infectious disease.

Anticipated hiring range: $77,000-80,000 annual, commensurate with education and experience.
