Every home tells a different story. As a home health nurse, you'll care for patients in the environment where they live their daily lives, allowing you to provide personalized care that addresses their unique needs and goals.

In this role, you'll:

• Build lasting relationships with patients and families
• Help patients recover, manage chronic conditions, and avoid unnecessary hospital visits
• Deliver one-on-one care with a focus on the whole person
• Enjoy greater autonomy and flexibility in your day
• Collaborate with an interdisciplinary team dedicated to improving outcomes

• Assess patient needs and develop individualized care plans
• Provide skilled nursing care and patient education in the home
• Collaborate with physicians, therapists, and care team members
• Monitor progress and adjust care based on patient goals and needs
• Support caregivers and family members with education and resources
• Complete documentation accurately and timely

• Current Registered Nurse license
• Ability to meet all state and organizational requirements for employment
• Valid driver's license and reliable transportation, as required for the position

• Home health experience
• Experience working with adult and geriatric patient populations

The salary range for this role is $82,995 - $124,492 annual total cash target pay based on full-time employment and may vary based on factors including, but not limited to, local labor markets, education, work experience, and certifications. Compensation and benefits information will be discussed during the hiring process.
