Description Intake Assessment Representative - Home Health Patient Access at Providence Home Services Oregon Schedule: Day Shift, Monday- Friday with rotating weekend coverage Job description: The Intake Specialist serves as the main access point for Home Health Services. This position is responsible for initiating and coordinating Home Health referrals, gathering all necessary referral and payor information and then inputting into EMR, verifying patient information and initial HH eligibility with patient and referral source, and coordinating and scheduling date for HH admission. This position ensures that information obtained at time of referral is accurate and complete. This position represents Home Health to customers and patients effectively promotes Home Services and products. Providence caregivers are not simply valued - they're invaluable.
