The Primary Care Network is looking for a Licensed Independent Social Worker to provide behavioral health care management for patients with chronic medical conditions in integrated primary care settings in Mason, West Chester, and Liberty Township. This individual will primarily provide assessments, diagnosis, treatment planning, case management, and individual and group therapies while meeting all standards of practice as required. This position is a hybrid work model with days on-site in the clinic and days working remotely. Essential Functions
Required Education
Master's Degree must be in Social Work. Required Trainings/Certifications
State of Ohio Licensed Independent Social Worker (LISW). Required Experience
Two (2) years of supervised social work experience. Physical Requirements/Work Environment
