Non-Paid Internship - Hub & Spoke Health Home

General Information on Population:

The Hub & Spoke Health Home program serves individuals 18 years of age and older. All individuals enrolled have identified substance use needs and may also have co-occurring mental or physical health conditions. To meet criteria for Hub & Spoke Health Home enrollment an individual must have or be eligible for BadgerCare Plus or Medicaid. If a person does not have health insurance or has minimal health insurance, staff at WCS can help that person determine if they are eligible for BadgerCare Plus or Medicaid.

Brief Description of Programs/ Project:

The Hub & Spoke Health Home program aims to connect individuals with services to help support their health and recovery. Once enrolled, an individual is paired with a support team that includes a Recovery Navigator, who understands substance use and recovery on a personal level. Recovery Navigators then act as a bridge and connect individuals to the appropriate behavioral and healthcare services to meet that person's unique needs. The Hub & Spoke Health Home team seeks out ways to strengthen each person's community, family, and social supports to help achieve the identified recovery goals. Examples of services include but are not limited to Peer Support, connection to housing/food, employment support, and transportation support.

A Recovery Navigator assists individuals with the creation of a personal treatment plan. They also assist individuals with making connections and follow-up appointments to turn the treatment plan into action.

Job Qualifications:

Characteristics Desired in Student Intern:

  • Interest in working with adults who are in recovery from substance use

  • Strong desire to build up the skills of others to improve their lives

  • Interest in facilitating support group(s) for adults

  • Interest in strengthening assessment skills via the utilization of tools such as the Brief Addiction Monitor (BAM) and other identified tools based on one's clinical needs

  • Willingness to enter data and intake/discharge documentation into appropriate database

  • Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth

  • Ability to address all situations from a trauma informed approach

  • Ability to be independent and self-directed; maintain accountability and responsibility

  • Eagerness to learn and willingness to ask any questions to enhance ability to perform job responsibilities and meet their own expectations of what they what to learn through the internship/practicum

  • Maintain client confidentiality within all written and verbal communications

  • Ability to use time efficiently and effectively

  • Strong organizational skills, ability to manage multiple complex tasks, strong professional communication and teamwork.

  • Flexibility in scheduling their time on site and within the community when needed

  • Working knowledge of what it means to be in recovery and the different pathways to recovery; patience and understanding of difficult life needs

  • Ability to interact with escalating adults in a calm and professional manner; ability to respond appropriately in crisis situations

  • Willing to receive critical feedback and implement any growth plans that on-site supervisor gives

  • Ability to follow oral and written instructions

  • Proficient with Microsoft Word and Excel; ability to both read and enter data in Excel

  • Perform basic computer functions

Training Requirements:

  • Students must be part of a Master's level social work or clinical program

    Health Requirements:

  • Students interested in internship/Practicum placement within the Hub & Spoke Health Home program do not need to meet any health requirements at this time; however, this could be subject to change.

Other:

  • Some travel in the community may be required, including but not limited to meeting team members at community locations for shadowing opportunities (intake assessments, follow-up visits).
