The HCBS Wait List Care Coordinator is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that a person receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community. Working Schedule: Schedule is Monday through Friday between the hours of 8 am to 5 pm. This position is a field-based position with a home-based office. You will work from home when not in the field. Location: Northeast KS - Johnson, Douglas, Wyandotte, Atchison counties. Local travel up to 75% and mileage is reimbursed at current government rate. Primary Responsibilities:
