Job Summary

The Medical Case Management Services of the Partnership Comprehensive Care Practice (PCCP) Outpatient Clinic serves an essential role as part of a multi-disciplinary team that provides patient care and education for adult patients living with HIV. Under the direction of the Medical Case Manager Program Manager, this position will provide a range of trauma informed and patient centered activities focused on improving health care outcomes. Duties include, but are not limited to Initial assessment of service needs, timely and coordinated access to community resources, and positively promotes the Mission, Vision and Values of the Partnership Comprehensive Care Practice and Drexel Medicine.

Essential Functions



  • Provide in person and telephonic services to patients diagnosed with HIV/AIDS not already enrolled in case management services or in immediate need when the assigned case manager is not present.
  • Respond to all clinical referrals and client requests for case management within the required response time.
  • Complete brief assessments to determine if patients can benefit from ongoing case management services (longer than 6 months), temporary case management services (under 6 months) or brief interventions.
  • Complete or assist in the completion of all referrals as requested by the patient or clinical team to ensure medical care is accessible.
  • Engage in dialogue to address any barriers that negatively impact health outcomes. This may include behavioral health, transportation, substance use, childcare, safe/affordable housing, medication adherence, and/or any specific challenges faced by marginalized populations. Coordination and linkage to needed community services.
  • Coordination with the Outreach team to engage lost to care patients and/or to prevent lost to care patients.
  • Create and maintain, in conjunction with the medical case management team, resource folders to be accessed by the MCM and Behavioral Health teams.
  • Maintain the weekly productivity requirements.
  • Document all services in the electronic medical record in a timely manner.
  • Enter all productivity units into the Careware Database.
  • Participate in the supervision sessions and team meetings as required.

Required Qualifications



  • Minimum of a Masters Degree in Healthcare, Social Work.

  • Minimum of a year experience or internship in a health care setting preferably in a HIV/AIDS organization.
  • Experience with mental health and addiction services preferred.
  • Familiarity with electronic medical record systems, and proficiency with standard computer systems, including Microsoft Office and Zoom are required.

  • Knowledge of and experience supporting diverse patient backgrounds, including race, gender, and LGBTQIA+ identities and deep interest in advocation for marginalized communities and improving diversity, equity and inclusion required.
  • Familiarity with electronic medical record systems, and proficiency with standard computer systems, including Microsoft Office and Zoom are required.

Additional Information

This position is classified as exempt grade K. Compensation for this grade ranges from $54,630 to $81,940Per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate's qualifications and experience, department budget, and an internal equity review.
