Description
- Organizes and conducts Physical Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities.
- The Physical Therapist directs the patient's participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.
- Performs evaluation(s) and develops an effective treatment plan appropriate to a patient's rehabilitation needs within the care continuum.
- Performs effective treatment techniques in a competent manner.
- Educates patient/family/caregiver in applicable health information, treatment and prevention.
- Initiates and participates in the discharge planning process.
- Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
- Performs other duties as assigned.
- Complies with all policies and standards.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- This position is in Home Care Services.
- 100% travel to the patients homes to provide care.
- Monday-Friday with rotating weekends and holidays.
- THIS POSITION WILL COVER THE CENTRAL MARKET WHICH INCLUDES AREAS AROUND CLEVELAND MEDICAL CENTER AND AHUJA.
- The territory for coverage is:
* , City of Cleveland, Garfield, Shaker Hts., Euclid, Richmond Hts., Cleveland Hts., Mayfield Hts., Beachwood, Pepper Pike, Lyndhurst, University Hts., South Euclid, Bedford, Solon, Maple Hts., Twinsburg, Hudson, Chagrin Falls, Macedonia, Northfield and Pennisula.
Sign-on Bonus: This position is eligible for a $6,000 sign-on bonus to eligible candidates. Eligible candidates must be newly hired to UH and have at least 1yr of PT experience. University Hospitals is offering a $6,000 sign on bonus, minus applicable taxes, for designated regular full-time PT positions to experienced PT's (1 years or more) hired externally. Internal Applicants are not eligible for the sign on bonus.
