I. Major Responsibilities:
1. Demonstrates knowledge of social casework theory and practices.
2. Demonstrates knowledge of bio-psychosocial factors related to illness.
3. Demonstrates knowledge of community health and social resources
4. Demonstrates knowledge of community health and social resources
5. Demonstrates knowledge of patient rights, including confidentiality, access to care, self-determination, and provider disclosure.
6. Demonstrates knowledge of legal decision-making process & options
7. Planning--attends all Interdisciplinary Patient Care Team meetings
8. Describes behavioral symptoms and observations objectively, in logical sequence, using proper English, terminology, and approved abbreviations
9. Uses effective verbal and nonverbal communication appropriately with others
10. Informs the Director of any and all discharge planning incidents which are barriers to effective treatment planning and care delivery

II. Position Qualifications:

License/Certification/Education:
Required:
1. Bachelor degree in Social Work, or related degree, required.
2. Graduate degree preferred.
3. Current Social Work license by the Massachusetts Division of Registration in Social Work.

Experience/Skills:
Required:
1. Minimum of one year experience in a health care setting, preferably in acute care.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

On-the-job time is spent in the following physical activities:
Physical Activity - Moderate physical activity required including, but not limited to walking. Must demonstrate emotional stability. Must demonstrate flexibility in relating to individuals of varying disabilities, and from different cultures, socio-economic and spiritual backgrounds.

This job requires exposure to the following environmental conditions:
Working Conditions - Good physical environment. May be required to work additional hours based on workload demands. Minimal travel for attending meetings and educational programs outside the hospital, and for home visits to patient families.
